Who is Eligible?

To receive this high-quality, dedicated service, give us a call. We serve patients from the southern end of Bend up to Madras, east to Prineville, and west to Camp Sherman and everything in between.

What is the Cost?

At ClearPath Healthcare, we believe in removing barriers to quality care. We bill Medicare, Medicaid, and most major private insurance plans directly, just as a traditional clinic would. You are responsible only for your standard co-pays and deductibles. We do not charge membership fees or travel costs for our in-home visits. About Our In-Home Primary Care Team

Our in-home primary care model is structured so that both our provider, Taylor Garr-Jones, FNP, and our Licensed Practical Nurse visit your home together for every visit.

Taylor is a board-certified Nurse Practitioner with a clinical focus in geriatric healthcare, hospice, long-term care, and in-home visits. She has experience treating patients in geriatric settings and managing chronic conditions, and she provides Medicare Annual Wellness Visits, labs, and ongoing coordination with your specialists. ClearPath provides primary care, the same kind of care you would receive at a primary care physician’s office, delivered to your home. Home health agencies are different. They typically provide short-term, physician-referred services such as:

  • Skilled nursing
  • Physical therapy

  • Short-term rehabilitation

Home health services usually require a physician referral and are often temporary. ClearPath focuses on ongoing primary care and long-term health management. When home health services are needed, we coordinate closely with your preferred agency so your care stays connected.

ClearPath offers comprehensive primary care in the comfort of your home, with longer appointments that allow more time to focus on your health goals. Typical visits last about 45 minutes and may include both a nurse and a clinician. Services may include:

  • Routine primary care visits
  • Chronic disease management
  • Medication review and management
  • Cognitive assessments and memory screening
  • Advance care planning
  • Basic wound care
  • Ear cleaning
  • Minor skin procedures, such as cryotherapy

Yes. ClearPath works with most Medicare, Medicaid, Medicare Advantage, and private insurance plans. Our team is happy to verify your insurance benefits before enrollment. Call our office at 541-323-0853 to confirm whether your plan is accepted.

Visit frequency is determined between you and your clinician based on your current health needs. Some patients are seen monthly, others more or less often, depending on what is going on with their health at the time.

No. There is no minimum time commitment. We would love to have you on our service for as long as you would like to stay with us. Life is unpredictable, and we believe your health and happiness come first. If you decide to stop services, we simply ask for a 30-day written notice.

Your insurance is billed for medical services just as it would be at a traditional clinic. You are responsible only for any co-pays or deductibles required by your insurance plan. ClearPath does not charge membership fees or travel costs for in-home visits.
